Indications

The yeast Malassezia (formerly known as Pityrosporum) is implicated in diseases such as pityriasis versicolor (localized), seborrhoeic dermatitis, and pityriasis capitis. Ketoconazole 2 percent shampoo is used to treat and prevent infections caused by Malassezia (previously known as Pityrosporum) (dandruff).

Pharmacology

Ketoconazole 2 percent shampoo is a broad-spectrum imidazole-dioxolane synthetic antifungal drug that alters the permeability of the cell membrane to prevent the in vitro development of the following common dermatophytes and yeasts: Trichophyton rubrum, Trichophyton mentagrophytes, Trichophyton tonsurans, Microsporum canis, Microsporum audouini, Microsporum gypseum, and Epidermophyton floccosum; yeasts: Candida albicans, Malassezia ovale (Pityrosporum ovale), and Candida tropicalis; and Malassezia furfur, the organism that causes tinea versicolor (Pityrosporum orbiculare).

Dosage & Administration

The affected areas of the skin or the scalp should be washed with ketoconazole 2% shampoo, which should be left on the skin/scalp for 3 to 5 minutes before rinsing.
Treatment:

  • Pityriasis versicolor: Once daily for 5 days.
  • Seborrhoeic dermatitis and pityriasis capitis: Twice weekly for 2 to 4 weeks.

Prophylaxis:

  • Pityriasis versicolor: Once daily for 3 days during a single treatment course before the summer.
  • Seborrhoeic dermatitis and pityriasis capitis: Once every 1 or 2 weeks

Contraindications

In individuals who have a known hypersensitivity to ketoconazole, it is not recommended.

Side Effects

The use of 2% ketoconazole shampoo on the skin is typically well tolerated. A local burning sensation, itching, or contact dermatitis (owing to irritation or allergy) may develop on exposed regions, as with other shampoos. Ketoconazole shampoo 2 percent has been linked to oily and dry hair in rare cases.

Pregnancy & Lactation

Pregnancy and breastfeeding are not contraindications for using ketoconazole shampoo 2% since ketoconazole is not absorbed through the skin after topical application.

Precautions & Warnings

To avoid a rebound effect in patients who have been on topical corticosteroids for a long time, it is suggested that the steroid medication be progressively tapered off over a period of 2 to 3 weeks while using ketoconazole shampoo 2%. Hair loss is frequently related with seborrhoeic dermatitis and dandruff, and has been observed seldom with the use of ketoconazole shampoo 2%. Avoid making eye contact. If the shampoo gets into the eyes, rinse them thoroughly with water.
